Willow Sage Services will not deny anyone essential services due to inability to pay, and offers discounts based on family size and annual income. 

Please complete the application below and return it to the front office to determine if you or members of your family are eligible for a discount.

If you require any assistance in completing the application, please call and ask for an Intake Specialist to assist you.

At Willow Sage Services, patients and parents/guardians have the right: to be treated considerately and respectfully regardless of the patient and/or family’s race, religion, sex, disability, sexual orientation, gender identity/expression, cultural background, economic status, education or illness, or ability to pay. Discounts for essential services are offered based on family size and income.
